From the North
Take Route 95 South to Route 195 East (Exit 20). Proceed on Route 195 East to Exit 4 (Route 44 East). Go through 4 sets of lights, and bear left onto Route 44 East. Bear left onto Route 44 West (Taunton Avenue) and take the first right onto Irving Avenue. Take the first left onto Alexander Avenue, and Linn Health & Rehabilitation is on the right. Parking is available on the street or in the parking lot behind the building.

From the South
Take Route 95 North to Route 195 East (Exit 19). Take Exit 6 (Broadway). Take a right off the exit. At the first set of lights, take a right onto North Broadway, go under the overpass, and take a right onto Alexander Avenue. Linn Health & Rehabilitation is on the left side of the street. Parking is available on the street or in the parking lot behind the building.

Visiting Linn Health & Rehabilitation
We have flexible visiting hours and do not restrict visitation. We encourage family and caregivers to visit as much as possible. The Main Entrance and exterior doors are locked at 8PM nightly.
