Take I-95 South to US-44 East/Taunton Ave. Use the right two lanes to take exit 36A for US-6 East. Continue onto US-6 East and then onto I-195 East/US-6 East.Take exit 1B-C for US-44 East/Taunton Ave. Go straight onto Waterman Ave, turn left onto Hall Street, and then bear left again onto Taunton Avenue. Take a right onto Irving Avenue and then in a couple hundred feet take the first left onto Alexander Avenue. Linn Health & Rehabilitation is further down the street on the right. From the South
Take I-95 North to I-195 East. Take Exit 36A to merge onto I-195 East/US-6 toward East Providence. Take exit 1B-C for US-44 East/Taunton Avenue. Continue onto US-44 East/Taunton Ave, then continue straight onto Waterman Avenue, take a left onto Hall Street, and turn left again onto Taunton Avenue. We have flexible visiting hours and do not restrict visitation. We encourage family and caregivers to visit as much as possible. The Main Entrance and exterior doors are locked at 8PM nightly.

Take I-95 South to exit 37A. Take exit 37A for Atwells Avenue. Continue onto John J Parington Way/Service Road 1, take a right onto Federal Street, then turn left onto Dean Street. Arbor Hill will be located on the left side of the street. Parking is available on the street or behind Arbor Hill, simply continue down Federal Street and to the right will be a gated parking lot.

From the South
Take I-95 North to exit 37A. Take exit 37A toward Broadway, then merge onto David Gavitt Way, and turn left at the light onto Broadway. At the second light, turn right and proceed onto Dean Street. Arbor Hill will be located directly at the corner between Federal Street and Dean Street. Parking is available on Federal street behind Arbor Hill, there will be a gated parking lot to the right.

Take RI-146 South to RI-146A South. Take exit 14 for RI-146A toward North Smithfield/Burrillville and keep left at the fork to continue toward RI-146A South. Turn left onto RI-146A South followed by a sharp left onto Mendon Road. Continue on Mendon Road and bear right onto Rhodes Avenue. Turn left onto 2nd Avenue and turn right onto River Street. Turn left and continue onto Blackstone Street, then turn left onto West School Street, then turn left again onto North Main Street, and turn right onto East School Street. St. Germain will be located on the right. Parking is available on the street or in the parking lot next to the building.

From the South
Take I-95 North to RI-122 North exit. Continue onto I-295 North, follow signs to Woonsocket. Take exit 18B toward Woonsocket, use the right two lanes to turn slightly right onto RI-99 North. Turn left onto RI-122 North/ Mendon Road, continue straight onto Cumberland Street. Turn right onto Rathbun Street, then turn left onto Elbow Street. St. Germain is located on the left of the street. Parking is available in the lot next to St. Germain along Elbow Street, or along the street.

Visiting Aldersbridge Physical Therapy & Wellness Center
We are housed in an 1,800 sq. ft. state-of-the-art suite within Linn Health & Rehabilitation and are open to residents and the public on an outpatient basis.
